Our office is open Mon-Fri 8:30 a.m.– 4:30 p.m. To better assist us, new patients should have the following information available:

Patient’s Social Security Number
Patient’s Address and Phone Number
Patient’s Diagnosis or Reason for Visit
Patient’s Referring Doctor Name, Address, and Phone Number

You should request that your medical records be forwarded to our office before your appointment including any MRI scans performed within the last year. It is important that you verify coverage with your insurance company before your visit to avoid unanticipated coverage problems. Many insurance companies require a written referral for patients seeing a specialist. If your insurance company has this requirement it is imperative that you work with your referring doctor to ensure that the written referral process is completed before your appointment.
